I hereby acknowledge that I have voluntarily registered for and intend to participate in the City of
Hillsboro 4th of July Parade presented by the Hillsboro Rotary Club. I understand that
participating in a parade can be a hazardous activity. I am aware of the risks and dangers
inherent with parade activities, and I knowingly assume the risk of property damage, personal
injury, or death that may result from such activities.
I agree and state that I am fully capable of participating in parade activities, and I have no
known physical or mental conditions that might hinder my safe participation in the parade. I am
fully capable of handling the potential hazards of a parade, including but not limited to, hazards
of traffic, weather conditions, and the safe and appropriate operation of any vehicle or other
prop, cart, wagon, trailer, float, horse, or other similar equipment I operate associated with the
parade.
In consideration for being permitted by the City of Hillsboro and the Hillsboro Rotary Club to
participate in the Hillsboro 4th of July Parade, I release from any legal liability the City of
Hillsboro, and Washington County including its officers, employees, agents, volunteers, elected
officials, and the Hillsboro Rotary Club, including its officers, members, and volunteers for any
and all property damage, injury or death caused by or resulting from my participation in the
parade activities provided by the City of Hillsboro, Washington County, or the Hillsboro Rotary
Club, whether or not such damage, injury or death was caused by the negligence of the City of
Hillsboro, Washington County, or the Hillsboro Rotary Club or any other cause or legal theory of
recovery.
I further agree not to sue, claim against, attach the property of, or prosecute the City of
Hillsboro, Washington County, the Hillsboro Rotary Club or any of their officers, employees,
agents, volunteers, elected officials, or Rotary Club officers and/or members and volunteers for
any property damage, personal injury or death caused by or resulting from my participation in
the City of Hillsboro 4th of July Parade, whether such damage, injury or death was caused by or
resulted from their negligence or any other cause.
I agree to defend, indemnify, and hold harmless the City of Hillsboro, Washington County and
the Hillsboro Rotary Club, including their officers, employees, agents officers, volunteers,
elected officials, or Rotary Club officers and/or members and volunteers for any property
damage, injury or death caused by or resulting from my participation in the parade activities,
whether or not the damage, injury or death was caused by or resulted from their negligence of
any other cause.
I agree this contract shall be legally binding on me, my estate, heirs, assigns, guardians and
conservators and personal representatives.
I have carefully read the above and fully understand its contents. In the event of a group parade
entrant, I represent that I have caused this release and indemnification to be communicated to
all individuals who will participate in the parade in my parade entry. I agree that this release and
indemnification shall be binding on all individuals who will participate in the parade in my parade
entry, their estates, heirs, assigns, guardians and conservators and personal representatives.
